• Will my Insurance cover chiropractic and massage?

There are many insurance companies that cover alternative care, every insurance is different. We are happy to check your benefit so that we know exactly what your policy covers.  

• What is my deductible?

You may be required to meet your deductible before your insurance will reimburse for your visits.  Deductibles vary based on your plan, we verify deductible amounts before your visits, so that you know what you will have to pay out of pocket.  

• What is co-pay and co-insurance?

A co-pay is the amount that you are responsible for at the time of visit, this is a fixed amount and it can range from 5.00 - 50.00 dollars.   Co-insurance is a certain percentage that is calculated on every code that is billed by the Dr.  The amount usually ranges between 10% and 30%.  

• What insurance companies are you contracted with?

Dr Fox is contracted with:  Regence BCBS,  Healthnet,  Providence, Aetna & Kaiser

Dr Crokin is not currently contracted with any insurance companies.

We do bill every insurance company, most companies do have out of network  coverage.  

Dr Fox and Dr Crokin accept patients who have been in a Motor Vehicle accidents and  Work related injuries.
